The ACCE Group Term Life/Accidental Death & Dismemberment Plan

The Life and Accidental Death & Dismemberment (AD&D) benefit plans are based upon annual salary.  Those eligible are all active employees working at least 17.5 hours each week, and their eligible spouses and children (up to age 19, or to 26 if they are full time students). 

Term Life and AD & D coverage options are:

Employer: (you choose one option for all employees)

  1. Option A:    1.5 x annual salary up to $500,000
  2. Option B:    2.0 x annual salary up to $600,000
  3. Option C:    2.5 x annual salary up to $500,000

Additional coverage includes:

  1. Spouse:      $5,000  (not to exceed 100% of employees coverage amount)
  2. Child:         $2,000

 

Benefit Features

Life:

  • Portability - If you retire, reduce your hours or leave your employer, you can take this coverage with you according to the terms outlined in the contract.  However, if you have a medical condition which has a material effect on life expectancy you will be ineligible to port your coverage.
  • Accelerated benefit (Life insurance) – a portion of the participant’s benefit will be paid prior to death if he/she becomes terminally ill.
  • Waiver of premium if participant becomes disabled for 180 days
  • Independent financial counseling for beneficiary
  • Conversion option upon termination of employment

AD&D:

  • Education benefit for school age beneficiaries of 12.5% of the total benefit or $12,500 (whichever is less)
  • Seat belt provision of an additional $10,000 if participant dies while wearing a seat belt
  • Independent financial counseling for beneficiary
  • Conversion option upon termination of employment
